Do I have to pay twice to sync between two devices?

In short no.

Any purchase made, whether on iOS or Android can be shared so log as you have created  a Baby Feed Timer account. When you log into this account from your other devices they will automatically be given access to the full version as well.

You can create an account by going to the "My Account' screen, which is found under the 'More' tab.
